Based on record review, the facility failed to protect the Private Health Information (PHI) of one Patient (A.) This failure caused a breach of Patient A's PHI.Findings:The facility Breach Report, dated 4/8/13, was reviewed on 4/9/13. The report indicated Registered Nurse 1 (RN1) had inadvertently placed the face sheet of Patient A into the transfer paper work of another patient. The face sheet contained the following information: Patient A's name, date of birth, age, sex, employer, insurance carrier and numbers, medical record number, and account number. The face sheet also had the next of kin and person to notify names and contact numbersThe information was destroyed.The facility investigated the breach, and provided training and counseling to RN 1.
