Issue:
VA Employee A faxed Veteran Bs lab and two radiology reports to the wrong number. The intended fax was to go to Campbell County Memorial Hospital with a prefix of 688 but instead the employee dialed 684 and faxed to the Buffalo Bureau of Land Management office. The Buffalo BLM called the employee to notify them of the mistake and the BLM Office has destroyed the records sent in error. It also turned out that the BLM employee was a former Sheridan VAMC employee. Update: 09/22/11:Veteran B will be sent a letter offering credit protection services due to full name and SSN being disclosed.
Outcome:
The mis-faxed information was destroyed. Also the information was faxed to the correct number.
