Issue:

Veteran A is enrolled at the Cookeville Community Based Outpatient Clinic (CBOC). On April 27, 2011, Veteran A contacted the pharmacy call center to request a refill on his medication. The request was referred to the Cookeville CBOC.On April 29, 2011, the LPN at the Cookeville CBOC, attempted to contact Veteran A at his phone number listed in the computer. The LPN spoke with the Veteran's daughter-in-law and relayed the following: The Veteran had failed his urine drug screen and would not receive a refill on his medication. Update: 05/02/11:Veteran will receive a letter of notification.

Outcome:

5/19/11: LPN completed additional privacy training. It was also discovered the documented information regarding this Veteran's health care proxy and disclosure instructions were unclear in that the Veteran provided more than one contact number. The Nurse Practitioner contacted Veteran to clarify how he would like future messages handled. Per Veteran's request, staff will contact him regarding significant health issues. If the Veteran is unavailable, the staff will leave their contact information for Veteran to return the call. Notification letter mailed to Veteran on 5/11/11.
