On 10/01/14, Veteran A reported to the VA Clarksville, TN Clinic to report that when he received a Patient Notification Letter of Test Results in the mail, he also received a Patient Notification Letter of Veteran B. Veteran A returned the letter belonging to Veteran B to the Clarksville Clinic, who then provided to the Privacy Officer.
Outcome:
10/01/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
