Issue:
An education class has been using a sign in sheet which lists patient names, last 4 of SSNs, length of diagnoses, and manner of treatment (i.e. insulin/pills). The sign in sheet listed PHI of nine patients. It appears that not only was this list being used, but also that the list was discovered in the location of the education class after the class had ended. At the time of submission of this ticket it is unclear whether the parties involved (reporter/generator of the roster) were contractors or VA employees. Update: 11/16/12:Nine patients will be sent a letter of notification.
