Issue:

A bio-hazard bag was found in an inpatient, locked, dirty utility room over the weekend. It looked like someone had emptied a sharps containers from 4 of the inpatient wards into this bag and then went through it looking for drugs. We found 5 medication labels that contained the patient's full name, last 4 of the SSN and the name of the inpatient ward. There were also two patient wristbands that contained the patient's full name and full SSN Update: 04/15/13:Two Veterans will be sent letters offering credit protection services and five Veterans will be sent a notification letter.

Outcome:

5 Notification letters and 2 credit monitoring letters were sent out to the patients involved. Redacted letters were put in the attachments section.The housekeeping Manager will be providing education to his weekend contract employee's on handling and disposal of the biohazard bags.Please close this ticket.4/26/13 - Please close this ticket.
