Issue:

A Veteran called the Privacy Officer to report that he received a box that contained medical equipment (CPAP) from the Miami VA that he did not request. He explained that his full social security number and date of birth were clearly visible on the outside of the box. He reported that his full name and address were also included on the outside of the box, but that that was necessary for the delivery of the medical equipment (although he did not order the equipment as he had just picked up the CPAP machine a few months before). The Veteran explained that he was returning to the medical center for an appointment and will bring in the box. Update: 01/25/13:Veteran A will be sent a letter offering credit protection services.

Outcome:

Medication box was retrieved and credit monitoring was provided to Veteran.
