Issue:

A Psychiatry Resident had his/her car broken into off campus and his/her backpack was stolen. In the backpack the resident had CPRS chart print-outs of 3-5 patients which included fill name, full social, diagnosis, and other personal information. They also had a list of approximately 30 patients which had patient name, last 4, and clinic they attended, ex. PTSD clinic. Update: 02/06/13:Up to Five (5) Patients will be sent letters offering credit protection services. Thirty (30) Patients will be sent a notification letter.

Outcome:

Action is being taken with the resident. Education is being provided to residents as a whole.
