Issue:

Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one patient (Patient A), when an employee inadvertently gave Patient A ' s armband and face sheet to the wrong patient.Findings:An interview was conducted on 8/9/12 at 11:00 AM, with the facility ' s Privacy Officer (PO). The PO stated that he became aware of a breach incident that happened on 7/23/12. The PO stated that an employee was discharging Patient A while having two computer screens up. The PO stated that the employee accidentally hit the wrong screen and incidentally gave another person to be discharged Patient A ' s arm band and face sheet. The PO stated that the breach was identified when the other patient was leaving and looked at the face sheet and noticed it wasn't his name. The face sheet and arm band was immediately returned to the nurse.Review on 8/9/12 at 11:00 AM, of the information disclosed in error contained the following Personal Health Information (PHI): Patient name Date of birth Age Address DiagnosisThe PO further stated that Patient A was notified of the breach incident.
