Issue:
A Program Support Administrator (PSA) working in Voluntary Services inadvertently disclosed 7332-protected information to a volunteer (a Veteran) when she handed the Volunteer an applicant's VA Form 10-0755. The applicant had indicated that he was HIV Positive in the "Restrictions, Limitations" block of the form. The PSA had been instructed to call the applicant and discuss their application. Instead, the PSA delegated this to a Volunteer, thereby widening the circle of knowledge of the applicant's 7332-protected information. While the Volunteer has had Privacy training, he did not have a need to know the applicant's condition, which was improperly disclosed when the PSA handed him the form and instructed him to call the applicant about his 7332-information. At this time the facility is checking to confirm whether or not the Volunteer applicant is in fact a Veteran. Update: 11/14/12:Veteran A will be sent a notification letter.
Outcome:
The employee has been entered into a personnel improvement plan (PIP).
