Issue:

Cardiology staff reported that a folder with print outs of cardiology consults and steno pad with list of follow up patients is missing from her desk. Steno pad only had patients still needing follow up. Any completed ones were removed from pad. At most she states there were 13 Veterans. Placed items in desk drawer at end of yesterday (desk does not lock) and locked office door. Office was locked this morning when returned. She has retraced her path from yesterdays to see if left somewhere. She has contacted EMS supervisor to see if EMS picked up on mistake. The 13 Veterans' names, dates of birth, full SSN and protected health information were compromised.

Outcome:

09/10/14: The Incident Resolution Service Team determined that the 13 Veterans will receive a letter offering credit protection services. 09/25/14: On further review only 10 Veterans were involved, and only three of them had full SSNs disclosed. Those three will be offered credit protection services and the other 7 will be sent HIPAA notifications.
