Issue:

Patient A received a copy of Patient B's non-VA medical records that were being returned to him in the mail. The non-VA medical records consisted of x-ray and ultrasound reports that included Patient B's full name, date of birth, and medical diagnoses. Update: 12/13/12:Patient B will receive a letter offering credit protection services.

Outcome:

Ambulatory Care Nurse Manager is working with staff to develop a process to be able to trace errors and identify which staff person made the error. The VA staff responsible for mailing patient information have been reminded that we are all responsible for protecting the privacy of our patients information and to take the appropriate precautions to prevent a reoccurrence of this type of incident.
