Issue:
Patient A received a Medline Industries medical supply intended for Patient B. Patient Bs name and type of medical supply was compromised. Patient A reported the incident to the medical center and a replacement has been requested for Patient B. Great Lakes Consolidated Mail Outpatient Pharmacy (CMOP) investigation concludes that this was a Medline packing error. The packing error has been reported to Medline for investigation and corrective action. Update: 01/11/11:Patient B will receive a letter of notification since his medication information was exposed.
