Issue:

A person walked into the File Room area and asked for his own medical chart for a compensation claim. A File Clerk gave the medical chart to the person without identifying who the person was and did not check out the chart to him. The entire Paper Volume 3 Chart is missing and has not been returned. Update: 10/12/12:The Privacy Officer (PO) contacted the Patient/veteran and asked him if he had come to the file room and requested his chart. He stated that he had not. The PO checked the DSS/ROI software system and no records were recently ordered for him. I checked as far back as December 2010. Due to full SSN and medical information being compromised, Veteran A will receive a letter offering credit protection services.

Outcome:

The employee is a newly re-assigned employee that has been restrained by his supervisor and the HIMS Chief. The credit monitoring letter has been signed by the Director and mailed to the Veteran.
