Issue:
A Pharmacy student was seeing a patient in Chronic Disease Management (CDM) clinic. When the student came to check out with supervisor with regards to the patient and possible medication changes, the student had the days patient list (which contained full name, SSN, DOB, telephone number and patient scheduled lab appointment information) and the patient medication list to write down medication changes for the patient. When the student returned to the patient, the patient when grabbing his medication list also grabbed the patient list and left with both. Once the student realized the list was gone, she informed her supervisor. The individual patient lives locally, was called and asked to please check the papers he had taken from clinic and if he found a patient list to please return it to the Team Clerks. The patient stopped by the following morning and dropped off the patient list to the Team clerks.
Outcome:
06/22/15: The Incident Resolution Service Team has determined that four Veterans will be sent a letter offering credit protection services.
