Issue:
Veteran committed suicide on VA grounds. Numerous employees have accessed this Veteran's record since this event. Letters questioning the reason for access were sent to supervisors of all who accessed this record. To date, inquiries regarding 43 employees have been sent, one for each of the 43 employees who have accessed this record. As of this date, 3 accesses have been deemed inappropriate. Responses have not yet been received for all inquiries. We are continuing to monitor access on a daily basis and follow up until responses are received regarding each access. Training/awareness/reminders are being provided as inappropriate accesses are discovered. Update: 09/19/11:The Veteran's Next of Kin will be sent a notification letter due to the Veteran's PHI and PHI being inappropriately accessed.
Outcome:
Administrative action will be initiated to those individuals who inappropriately accessed the medical records.
