Issue:

Veteran A was notified by the spouse of deceased Veteran B they had received VA paperwork belonging to him. The spouse of Veteran B was going through the VA mail when they discovered the paperwork belonging to Veteran A. The spouse was unsure how long they had the paperwork belonging to Veteran A, but indicated Veteran B died in August. On September 8, 2014, the spouse of Veteran B met with Veteran A and returned the VA paperwork which consisted of five pages. Veteran A contacted the facility Privacy Officer to report the event. The facility Privacy Officer determined the five pages was Veteran A's disability claim submitted to the Veteran's Service Representative on August 29, 2014. The documentation contained PII to include financial institution information and bank account numbers. The Veteran's Service Representative was notified of the event.

Outcome:

09/10/14: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
