Issue:

A provider gave a patient a piece of paper but did not realize there were several other pieces of paper attached containing about 10 Veterans progress notes, appointment lists containing appointments and full SSNs. The provider realized it about 15 minutes after the patient left his office. The provider was able to find the patient who reported he had disposed of the papers in a public waste can in a bathroom. The papers were found by VA staff. It is not believed the information was taken or misused. Update: 04/11/11:Ten Veterans will be sent letters offering credit protection services.
