Issue:

The son of a deceased Veteran made a written request for his fathers medical records and included a copy of the death certificate. Also, included with the request from the son, who is also a Veteran, was his personal information; a copy of his birth certificate, SSN Card and Drivers License. The request and the attached documents were sent via certified mail. The documents were signed for by the file room staff but the request for the medical records was not processed and the only document that can be located is the death certificate of the deceased Veteran (scanned document). The certified mail was signed for on July 7, 2014 and the Death Certificate has the same date stamped on it from our facility. After a thorough search the request and the support documentation could not be located. Privacy will further investigate.

Outcome:

10/16/14: The Service Chief has reported that after an exhaustive search, the son's documents cannot be found. The facility believes they would have been shredded but cannot determine that for sure. The Incident Resolution Team has determined that the son will be offered credit protection services, since his documents cannot be located.
