Issue:

A Veteran's wife called the Chief of Fee Basis and informed him in September or early October she had received another Veteran's "Request For Outpatient Services Form." She said she had tried to contact someone at the VA on several occasions but was unsuccessful. She did not provide the name of the other Veteran whose form they received and has not returned the form. She is under the impression someone else might have received their form. A check showed that the Request for Outpatient Services was mailed to the Veteran July 31, 2013. She said they didn't receive it and a check showed the Veteran hasn't had the test performed. She is requesting credit protection. Have requested the Chief of Fee Basis contact her to get the mismailed form returned.

Outcome:

01/17/14: The Privacy Officer believes that both Veterans had their information disclosed improperly. Both will be offered credit protection services.
