Issue:

The Community Outpatient Services (COPS) Nurse stated Veteran A received Veteran B's lab results. The COPS Nurse spoke to an Information security Officer (ISO) before contacting the Privacy Officer (PO) and stated the ISO told them to tell Veteran A to shred the lab results. At that time the PO explained to the COPS Nurse that shredding information concerning a privacy complaint is not an option for several reasons. The COPS Nurse stated that she will contact Veteran A to see if information was shredded. The PO advised her to tell the patient to bring the lab results of Veteran B back to the clinic or mail it if Veteran A did not dispose it. Update: 01/30/13:Patient B will be sent a notification letter.

Outcome:

Notification Letter was mailed and uploaded. Employees were counseled on 2/1/2013.
