Issue:
Today, 05/06/15, the Privacy Officer (PO) was provided with a privacy incident report per Veteran report on 4/17/15. The Veteran originally reported the privacy incident to Pharmacy when he sought 2 requested medications. Review determined that the Consolidated Mail Outpatient Pharmacy (CMOP) sent 2 prescriptions to the Veteran on 04/15/15 with package delivery on 04/17/15. The Veteran indicated that facility address on file was incorrect, and that he never lived at noted address. The address had been changed by facility staff on 04/13/15, for an unknown purpose. Veteran Complaint Rights and complaint acknowledgement letter will be provided to Veteran per new VHA policy. Additional notification and investigation, to ensue.
Outcome:
05/06/15: The Incident Resolution Service Team has determined that the Veteran will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
