Issue:

On August 9, 2012, @ 4:30 p.m., Veteran A called Medical Center Alt. Privacy Officer, and reported having been discharged from the Medical Center's PTSD Residential Rehabilitation program, at 11:30 a.m., 7/27/2012., and that a second Veteran - Veteran B, with whom Veteran A had become familiar while in the program, was also discharged from the same program at the same time. Both were issued copies of VA medical records, to which an appointment reminder card was stapled.Veteran A reports that, upon his return home to Indiana, he discovered that Veteran B's medical records were stapled to Veteran A's appointment reminder. Veteran A had exchanged phone numbers with Veteran B, and, about 12 and 1/2 hours following discharge - at midnight July 28, 2012, texted a message to Veteran B. Veteran B responded that Veteran A's medical records were attached to Veteran B's appointment reminder.Veteran A has secured Veteran B's discharge paperwork in his gun safe at home, however, feels that his own medical records may be at risk as Veteran B may be homeless. Update: 08/10/12:Veteran A and B will be sent a letter offering credit protection services.

Outcome:

CM Letters have been sent to the Veterans involved with this incident. Service Chief has educated the employee responsible.
