Issue:
The unit reports for units MICU, 9A, 7B, 6B, and 6A were not delivered to the correct location at midnight on 11/08/11. These documents contained 17 patients' full name, last four digits of the SSN, and notes such as "restraints", "committed", and "sitter." The documents were supposed to be delivered by the Escort service to the off-tour nursing coordinator in C5013 but were discovered missing on the morning of 11/09/11. Attempts were made by the nursing service to locate the documents without success. The MICU ward clerk found the documents inside a magazine in the MICU waiting room on Sunday, 11/13/11. Update: 11/16/11:Seventeen (17) Patients or Next of Kin will receive a notification letter.
Outcome:
Contact has been made with the vendor who provides Escort services. The COTR plans to identify and remove all Escort tasks requiring the handling/transporting of documents that contain protected health information within the next 3 weeks.
