Patient A was given medication belonging to Patient B. This happened in March and Patient A is just now returning the medication. Both patients were on the same medication, same dosage, and had the same last name; Patient A thought the wrong first name had been typed on the label.Patient A no longer has the packaging so the facility is unsure of Patient B's address; however, the last four of Patient B's SSN was written on the Rx label. Update: 06/14/12:Patient B will be sent a notification letter.
