Patient A received her own records that she had requested. Patient B's records were also included. The PO is not certain what records were disclosed nor who was responsible. Patient A is to return the records to the PO. Update: 01/31/11:The PO is waiting to receive the records back from Patient A.02/01/11:Due to the fact that Patient B's fulll SSN and PHI was exposed, Veteran B will receive a letter offering credit protection services.
