Patient A was given Patient B's discharge instructions by mistake upon discharge. Patient A will mail the discharge instructions back to the Privacy Officer (PO) . The discharge instruction contained patient B's name, address, date of birth, full SSN and protected health information. Update: 04/11/12:Veteran B will be sent a letter offering credit protection services.

Employees reminded of the need for caution when handling documents with PII.
