Issue:

Medications were prepared for 2 patients with the same name and the medication for Veteran A was passed to Veteran B. When Veteran A came in to pick up his medications the mistake was discovered. Veteran B was called and immediately brought in the incorrect medications and was given his own. Veteran B did not take any of the incorrect medications and no narcotics were involved. Update: 11/14/11:Veteran A will be sent a letter of notification.NOTE: There were a total of # Mis-Handling incidents this reporting period. Because of repetition, the other # are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

The pharmacy staff is putting together a team to evaluate all processes within the outpatient pharmacy department to minimize the potential for future breaches. Unfortunately, there has been no specific trends regarding specific staff or processes that have led to these events. We take these events very seriously and have reiterated to staff that these occurrences are not acceptable, and will do everything we can to prevent future occurrences.
