Issue:
Clinician reported that upon referral for nursing home placement for Veteran, a couple documents were mistakenly faxed to another business. The business notified us stating first page was the our fax cover sheet. The second page sounds like the first page of our cover-demographic sheet, but unsure. It was reported that the fax printed very small with lots of checked boxes and was difficult to read. A carrier pre-paid; pre-addressed envelope was sent for the return of the information in order to accurately report. The information will be returned tomorrow via their usual pick up time and until then will be kept secure.
Outcome:
09/03/14: The Incident Resolution Service Team has determined that the Veteran will be sent a letter offering credit protection services.
