Issue:

Veteran A was discharged to a non-VA nursing home on 12/16/2011. Relevant paperwork necessary for nursing home admission (medical progress note) was sent with the family of Veteran A as they transported him to the nursing home. Paperwork contained a progress note of Veteran B that was mistakenly included in Veteran A's packet. The Non-VA nursing home called to report the incident and were instructed to destroy the records of Veteran B. However, it is not known what, if any, information Veteran A's family might have seen or had access to. Update: 12/19/11:Veteran B will be sent a letter offering credit protection services, since Veteran A's family was given documentation that included Veteran B's name, full SSN, medical information, and date of birth.

Outcome:

Provided education/training/awareness to employee who disclosed documents. Also, chief of the service plans to remind everyone of privacy standards at next staff meeting.
