Issue:

On February 3, 2011, Release of Information Office received a returned Fedex box. The box contained a Veterans medical records (total of 551 pages). The records were divided and placed in two manila envelopes. Upon opening the returned Fedex box, one of manila envelope was torn open. The request was signed by the Veteran and sent to the correct address on August 11, 2008. Mail room informed the Privacy Officer that there is no senders information. Fedex cannot provide a tracking log that goes back to 2008. Update: 02/04/11:Veteran will receive a letter offering credit protection services since his full SSN may have been exposed.
