Issue:

A Veteran contacted his VA health care provider to report a fax number on the VA business card provided to him is incorrect. The Veteran who used the wrong fax number was contacted by the receiver of the fax and told of the faxing error. The receiver of fax stated he destroyed the document he received in error. The VA provider confirmed the business card has an error in the fax number. VA health care provided that reported the incident immediately creased in the distribution of her VA business card. Privacy will further investigate.

Outcome:

12/24/13: According to the provider: "I received the business cards after mid-September and have been handing them out to patients from that time until 12/20/2013. I would estimate approximately 125 have been given to patients." To date only one Veteran has advised they sent a fax to wrong number.
