Issue:
A VA employee reported today that he inadvertently gave Patient Bs appointment letter to Patient A. The employee printed out an informational packet for Patient A and didnt realize that Patient Bs appointment letter was within the contents. Patient A mailed Patient Bs appointment letter back to the employee and also mailed a copy to Patient B. The employees supervisor will be notified. The employee will be changing his printing process by printing to a closer more controlled printer. the appointment letter contained Patient B's name, address, partial SSN and appointment information. Update: 02/24/12:Patient B will receive a letter of notification.
