Issue:
Patient A prescription was found loose by the mail distributor. Patient A's name and type of medication was compromised. The mail distributor returned the prescription to the Dallas Consolidated Mail Outpatient Pharmacy (CMOP). According to documented process events, the order was packed and completed. Even though this process was documented complete, the final step of ensuring a proper package seal was not done. As a result, some of the package contents spilled out during handling. The CMOP properly packaged the item and reshipped it to Patient A. Employee will be retrained in proper packaging procedures. Update: 02/17/11:Patient A will receive a letter of notification since his medications information was exposed.
Outcome:
Patient Notification Letter mailed on 3/23/2011. CMOP packer has been counseled and retrained in proper packing procedures. Please close this ticket.
