Issue:
Veteran A reported to the Lee County Healthcare Center that he received a Veteran B's Veteran Health Identification Card (VHIC) by mail in error. The envelope was addressed to the other Veteran who bares the same first and last name. Veteran A returned the VHIC card. Veteran B is deceased.
Outcome:
09/25/14: The Incident Resolution Service Team has determined that the family of Veteran B will be sent a next of kin notification letter.
