Veteran/Patient A returned a Patient Payment History Report that was mailed to him and belonged to Patient B. The report includes Patient B's full name and full SSN. Update: 03/06/12:Patient B will be sent a letter offering credit protection services due to full name and full SSN being exposed.
Outcome:
Credit monitoring letter mailed March 12, 2012. CPAC supervisor notified of mis-mailing. PO will provide a follow-up Privacy Training for CPAC staff.
