Based on interview and record review, the hospital failed to provide confidential medical treatment for one of one sample patient (1). Findings:Patient 1 had a mammogram at one of the hospital's outpatient clinics on 5/29/12.On 8/24/12 at 9 a.m. during an entity reported investigation the privacy officer stated that Patient 1's mammogram information was sent to another patient who had the same first and last name, and same middle initial as Patient 1. The disclosed information for Patient 1 included her name, medical record number, date of exam, and information regarding the mammogram results.
