Issue:
In addition to his own test results, Veteran A also received Veteran B's test results in the same envelope. Veteran A contacted the Director's Secretary about the problem who in turn notified the Privacy Officer. Veteran A has been instructed to mail the letter and envelope to the Privacy Officer for review. Information compromised consisted of Last initial, last 4 SSN, name, and address, with a single line statement of test results. We believe this is connected to SPE's 73792 and 73797, in which a batch job of test results were machine-folded and stuffed by mailroom's Pitney-Bowes machine. The error occurred due to static cling associated with the paper. Pitney-Bowes has been notified. Update: 04/06/12:Veteran B will receive a letter of notification.
Outcome:
Pitney-Bowes contractor alerted, along with Service chief. Mailroom personnel asked to fan/flip paper prior to machine insertion, per contractor.
