Issue:

Based on staff interview and record review, the facility failed to maintain privacy and confidentiality for one patient (Patient A), when an employee inadvertently entered the wrong insurance information which resulted in the wrong insurance company receiving Patient A ' s information.Findings:An interview was conducted on 1/26/12 at 8:30 AM, with the Facility Privacy Health information Officer (PHO). The PHO stated that on 1/13 2012, she was notified that Patient A ' s insurance information was sent to the wrong insurance company. The PHO further stated that the breech of information was noted when Patient A was re-billed for non-payment.Review of the information disclosed in error included the following Personal Health Information (PHI). Patient name Address Insurance informationThe PHO further stated that Patient A was called and notified of the incident. The employee was educated and disciplined for the error.
