Report ID: PSETS0000089227, U.S. Department of Veterans Affairs
Reported Entity: ROSEBURG HCS - 653
Issue:
A manager was told that a copy of her SF-52 was posted to a poster board in the community break room in the department. She had printed two copies to a community printer (as individual printers have been removed from that department) and apparently someone took one of the copies before she got there. One of her supervisors then found the document tacked to the poster board, took it down and returned/reported the document the manager. Neither are sure exactly how long it was there, or who put it there. The document, contains the individuals Vet status (Manager is not a Veteran), full social security number, date of birth, and full name.
Outcome:
05/14/13: The manager will be sent a letter offering credit protection services due to full name and full SSN being exposed.
