Issue:
The Release of Information (ROI) Clerk from another hospital mailed a patient inquiry package using the address for the new hospital, which is still under construction. Clerk used Google to research the address and failed to note that the hospital is nearly 18 months from opening. The envelope was subsequently delivered to the on-site construction firm, whose secretary in turn handed it over to the hospital's Associate Director (AD) during one of his on-site meetings. The AD relayed the envelope to the facility Privacy Officer for resolution. Update: 07/26/11:Veteran A will be sent a letter offering credit protection services, due to full name and SSN being exposed.
Outcome:
ROI clerks and supervisor that mailed the letter to wrong address have been called and reminded not to use construction address for the new hospital untill officially opened.
