Issue:

Up to 66 patients may have received another Veteran\xe2\x80\x99s Request for Outpatient Services form that included the Veterans' names, address, date of birth, and last four of their social security numbers. A volunteer was assisting a VA Employee with creating packets of information for patients to make a non-VA Optometry appointment and accidentlly failed to correctly match the correct form to the letter in the packet before mailing out. It is currently unknown if all 66 patients were impacted or just a few. Update: 02/25/13: Up to 66 VA patients will receive a letter offering credit protection services. NOTE: There were a total of # Mis-Mailed incidents this reporting period. Because of repetition, the other # are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
