Issue:

Veteran A received Veteran B's medication and medication paperwork. The prescription was filled and dispensed on 10/30/14. The error was discovered on the same day. The dispensing Pharmacist realized the error and immediately called Veteran A to explain the situation and requested he return the mis-dipsensed medication. All content (medication and paperwork) was returned to the VA later in the day on 10/30/14.

Outcome:

11/04/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
