Issue:

Thirty-one pages of billing information (for another VA Hospital) was released to the Office of the Attorney General in Indianapolis, Indiana. This consisted of information on two (2) people. One person had her full social security number, full birth date, ICD-9-CM codes for her diagnoses, dates of service, claim number, and payment amount disclosed. The 2nd person had first and last name, claim number, payment amounts, service dates, home address, diagnosis, and ICD-9-CM codes disclosed.The Indianapolis Regional Counsel is writing a letter to the Attorney General to have the inappropriately disclosed information mailed back to the Privacy Officer. Update: 11/3/11: The Attorney Generals office, who received the Veterans' information, has no relationship with the Veterans or the VA. One Veteran will receive a letter offering credit protection services due to full SSN and DoB being inappropriately disclosed, and one Veteran will received a letter of notification due to PHI being inappropriately disclosed.

Outcome:

Education is on-going for the ROI clerk, disciplinary action has been taken and the employee has been placed on a PIP.
