Issue:
An individual received Veteran A's statement of medical care cost recovery account activities for December 2011. It was from the North Little Rock Agent Cashier. The individual called the Health Resource Center (HRC) to report this incident and then was told to mail the letter in which he did. The Privacy Officer (PO) checked in system and the Veteran's address is correct. The letter contained Veteran A's name, address, and medication information. Update: 01/09/12:Veteran A will receive a letter of notification.
Outcome:
PO has been in contact with several services and was unable to determine exactly who mailed this out to this individual. Staff in all areas have been reminded that they should always verify the address before mailing out. There is no such address in the system for this individual letter has gone out...will upload.
