Issue:

The Director's Office sent a C-File with our VA employed van driver to drop off the chart to the mail room. The chart was to be sent back via UPS to Washington D.C. They called us to state they never received the record. When the mail room pulled up the UPS reports there was no record of anything being sent to that address during the timeframe. The chart is now missing and staff have been notified to be on the lookout for a C-File. Update: 03/28/12:One (1) Veteran will be sent a letter offering credit protection services due to full name and full SSN in the documents that are missing.

Outcome:

We are looking at persons to hand deliver C&P Files to the mail room and waiting for the UPS tracking label instead of giving them to our VA Employed Courier. The courier goes so many places and dropped off the Cfile to the C&P department instead of the mail room for shipping. The C&P department shipped the chart back to Washington DC using UPS.
