Issue:

Leavenworth CMOP received a report from Ft Harrison VAMC stating Patient A received a prescription intended for a Patient B. Patient B received a prescription intended for Patient C. The patients' name and type of medications were compromised. Consolodated Mail Outpatient Pharmacy (CMOP) investigation concludes that this was a CMOP packing error with labeling UPS packages. The prescriptions have been resent to all patients. The packers willl be retrained on proper UPS labeling procedures. Update: 08/03/11:Patients B and C will be sent letters offering credit protection services.

Outcome:

The packer was retrained in proper packing procedures.
