A VA Employee/Veteran was mailed (within the facility, red bag) information pertaining to programs for spouse that are offered to Operation Enduring Freedom/Operation Iraqi Freedom (OEF/OIF) Veterans. The red bag was opened but not by Employee/Veteran, who it was addressed to, and then placed in Employee/Veteran mail slot unsecured. The Veteran's name and diagnosis were compromised.

Outcome:

06/05/14: The Incident Resolution Team determined that the Veteran will receive a HIPAA letter of notification.
