Issue:
Veteran A called the VA Chattanooga Outpatient Clinic to report that when he was at the VA Chattanooga Outpatient Clinic on Friday, January 11, 2013, he was provided various paperwork when leaving. When he got home, he then found he was also was given the paperwork of Veteran B.Veteran A did not and does not want to look at the document of Veteran B.PO contacted Veteran A and arranged for the document to be returned to the PO. PO is unable to determine, at this time, what the document was or what type of sensitive information it contained until receipt of the document. Update: 02/11/13:Veteran B will receive a letter offering credit protection services.
Outcome:
It was determined this incident was caused by a human error, in that the Nurse failed to perform a check of the documents, prior to providing to Veteran A.Nurse Manager was notified and the individual nurse involved will be provided education and additional training concerning checking documents. Nurse Manager will also discuss in their next staff meeting.
