Issue:

A physician gave a routing sheeting to Veteran A which belonged to Veteran B. The nurse realized the mistake and contacted Veteran A at home and Veteran A told the nurse that he would shred the information in his personal shredder. Update: 10/03/11:One Veteran will be sent a letter offering credit protection services due to full name and full SSN being disclosed.10/05/11:PO states this one shouldn't have been entered as an incident. Turns out the records never left VA control. They were mailed from the Waco regional office to the VAMC in el paso. They've been returned to waco and the Veteran's file has been reviewed to ensure there are no "mixed up" records in his file. No credit monitoring letter was sent to the Veteran, however the team responsible for mailing the copies to el paso has been counseled.

Outcome:

Provider was to make sure that he is given documents to the correct patient. Patient received credit monitoring.this was closed 10/28/11.
